Transitions Social Worker – Camden – £32p/h

Social Personnel are looking for an experienced Social Worker to work in Camden’s Transitions Team. The Transition team sits in the All Age Learning disability division brings together the following teams : Children With Disability team 0-18, a Transitions team 18-25, Learning disability social care team 25+ and specialist adult community learning disability health team who are working towards a seamless service.

Duties:

In this transitions role  you will carry out assessments based on the Care Act 2014, working in partnership to develop person centred support plans to meet the assessed needs. They will also carry out robust reviews of care packages ensuring that there is effective use of public funds.

What you need:

• You must be able to demonstrate that you are skilled in managing complex cases, preferably with disabled young adults. The manager is looking for Social Workers who have a more adults based background.
• Your experience of working in a statutory setting will allow you to operate with confidence and emotional intelligence – making effective assessments, implementing plans, delivering interventions and undertaking court work where required.
• QSW Status – including qualification, Social Work England registration and completed ASYE.
• Right to work in the UK.
