Social Worker – Older people & Physical Disabilities – Kent – £30p/h

Social Personnel are currently looking for a Social Worker to join Kent’s OPPD Service. They are paying a rate of £30p/h and the main focus of this role is to support Older people with physical disabilities.

Duties:

• Assessment and identification of memory difficulties associated with Dementia using various tools
• Arranging methods of intervention in arresting the spread of the disease through cognitive stimulation activities for social prescription
• Working in partnership with carers and significant others in monitoring developments after prescription
• Arranging Care Act Assessments for those whose impact is debilitating and needing both homecare respite and long-term packages which may include residential care.
• Reviewing packages of care to ensure continuity of service provision
• Working in partnership with OTS for any improvements in the living environment to promote independence
• Giving advice on healthy lifestyles to slow the progression of the disease.
• Doing clinical reviews with clients to assess progress on both the medication and other social interventions

What you need:

• QSW Status – including qualification, Social Work England registration and completed ASYE
• A minimum of 2 years experience in adults-based teams.
• Right to work in the UK.
