£38 – £40 per hour
Are you an enthusiastic and passionate worker who wants to give Powys residents the best chance of living independently after they have left hospital? Do you get the importance of a strengths-based approach to practice to enable people to do what matters to them? If so, we’ve got an exciting opportunity for you to join the newly created Powys Hospital Adult Services Team. You’ll be co-located with an existing integrated hospital team in Hereford and your role will be focussed on assisting patients to get the right care and support needed to maximise their recovery potential after leaving hospital. This role is unique in that you’ll be operating in England, but supporting Powys residents. You’ll be a crucial link between Powys Adult Services and the Hereford Hospitals.
The Powys Hospital Adult Services Team is newly formed and works hard to support people to live their own lives and help them achieve what matters, in line with the Social Services and Wellbeing Act (Wales) 2014.
We are looking for a highly motivated, reflective and creative professional with a social work or suitable health qualification to join this team. You will be a strong team player with great communication skills and a commitment to working collaboratively across our health and care system. You will need to be registered with Social Care Wales for this role.
