CHC Social worker –Gloucestershire – £35p/h – Remote based

Social Personnel are currently looking for an experienced adult social worker for a role within a continuing healthcare team based in Gloucestershire. The successful candidate will ideally have knowledge of the continuing healthcare process and experience of completing decision support tools in a multi-disciplinary meeting is preferable.

REMOTE WORKING

Duties:

• Picking up referrals when CHC funding has ended, as a result of application of the Decision Support Tool (DST)
• Putting support plans in place
• Working in a fast paced environment as this is time-limited work
• Working primarily with older people, as well as some younger adults too
• Carry out DSTs as part of a multi-disciplinary meeting alongside individual commissioning nurses from CCG CHC team.

What you need:

• QSW Status – including qualification, Social Work England registration and completed ASYE
• A minimum of 2 years experience in adults-based teams.
• Right to work in the UK.
