Social Personnel are looking for a Qualified Social Worker to join a Continuing Healthcare Team in Southend.

Duties:

  • Identify individuals who may be eligible for NHS Continuing Healthcare and refer them to the Clinical Commissioning Group (CCG).
  • Act as the Council’s representative at multidisciplinary team meetings. Provide evidence based recommendations on an individual’s eligibility for NHS CHC or other types of funding.
  • Provide written reports which are concise, evidence based, and to a standard which would enable them if required to be used for legal purposes.
  • Provide assessments/reviews which clearly outline an individual’s needs and can be used to assist the council in making a decision on an individual’s eligibility for NHS CHC.
  • Keep written records in line with Council Policy and professional standards. Ensure responses are provided within the agreed local time scales.
  • Demonstrate specialist knowledge in relation to NHS CHC and associated policies and legislation when asked to provide advice or represent the Council.
  • Demonstrate the ability to make independent decisions and support situations in which risk, ambiguity or complexity is present.
  • Ensure service users and their families understand the NHS CHC process, manage expectations jointly with the council and ensure that individual’s rights are upheld. Including the provision of advocacy where appropriate.
  • Support individuals to retain choice and control over their lives, by providing advice where required on Personal Health Budgets.
  • To act as a mentor/coach to Council Staff facilitating their learning and development in relation to NHS CHC. In doing so act as a link for particular Council Teams.

What you need:

  • Have post-qualified experience
  • Be eligible to work in the U.K.
  • HCPC registered and have a DBS.
  • A minimum of 2 years experience in Adults Knowledge of the Care Act, Mental Capacity Act, self-directed support, Liquidlogic, NHS Continuing Healthcare and experience of undertaking safeguarding investigations, which is essential.
  • Completed ASYE
