Adults Senior/Social Worker – Adults Hospital Team – Surrey

Our client, Surrey is seeking a Senior Social Worker/Social Worker to join their Adults Hospital team

What the role entails:

Social Worker

  • Integrated service to support people at home and avoid a hospital admission where possible.
  • Arrange support for people being discharged home which would include the Discharge to Assess scheme and supporting the person through the Care Act assessments and help with planning long term care if required.

Senior Social Worker

  • Work with the community services.
  • Confident assessment skills,
  • Knowledge of the relevant legislation such as the Care Act and the Mental Capacity Act and people who also want to supervise and support staff as well as shaping the team in this senior role.

Requirements of the role:

  • Qualified Social Worker
  • SW England registered
  • Must have completed ASYE
