Job Description

Social Personnel are recruiting an adults senior practitioner specialist lead for a CHC team based in Leatherhead, Surrey.


  • You will have a varied role that includes undertaking the team’s core duty function, a number of complex cases and NHS Continuing Health Care disputes, but your key role will be to provide professional support and advice for staff and managers in the locality, children’s and transitions teams.
  • You will provide coaching and training to help them maintain and raise the standard of work and equip others to do their work more efficiently and effectively. You will provide professional advice to social care staff around NHS Continuing Health Care and inter-related processes and legal frameworks relating to both adults and children; helping them prepare dispute rationales and briefings for managers as required.
  • You will be required to work in both community and hospital settings across Surrey. You will receive regular support and supervision and will provide support and professional supervision to social workers in the team.

What you need:

  • An understanding of the Forensic Social Work Capabilities.
  • QSW Status – including qualification, HCPC registration and completed ASYE
  • To have experience of hospital discharge process.
  • Right to work in the UK.
